Job Summary

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at CenterWell Senior Primary Care. As a Field-Based Nurse Practitioner, you will provide proactive, preventive care to seniors, including wellness visits, physical exams, chronic condition management, screenings, and minor injury treatment.

Key Responsibilities
  • Provide high-quality, patient-centered care to seniors in their homes, focusing on personalized experiences and addressing the factors that impact patient well-being.
  • Work collaboratively with integrated care teams, including physicians, nurses, and behavioral health specialists, to deliver compassionate and effective care.
  • Conduct wellness visits, physical exams, and chronic condition management, as well as screenings and minor injury treatment.
  • Develop and implement personalized care plans, taking into account the unique needs and circumstances of each patient.
  • Stay up-to-date with the latest medical research, guidelines, and best practices to ensure the highest level of care for patients.
Requirements
  • Valid state driver's license, clean driving record, and proof of auto insurance.
  • Bachelor's degree in nursing or a related field.
  • Minimum 2 years of clinical experience, preferably in primary care or gerontology.
  • Active, unrestricted state license, DEA license, and board certification as a Nurse Practitioner.
  • Strong technical skills, including proficiency in electronic medical records (EMRs) and other healthcare software.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
Preferred Qualifications
  • Master's degree in nursing or a related field.
  • Experience in house calls, urgent care, primary care, family practice, or internal medicine.
  • Knowledge of value-based care and HEDIS quality metrics.
