As an Advanced Practice Nurse at GuideStar Eldercare, you will play a crucial role in alleviating the challenges faced by an aging demographic suffering from dementia, Alzheimer's disease, and various neurocognitive disorders within nursing homes and assisted living environments.

Key Responsibilities:

Travel daily to designated facilities from your residence.

Conduct initial assessments, ongoing therapy services, and cognitive evaluations.

Document the necessity and scope of services for each patient.

Foster and maintain positive professional relationships with client facilities, patients, and their families.

Qualifications:

Proof of state licensure

Successful completion of pre-employment background check and drug screening.
