Key Responsibilities:
  • Conduct comprehensive assessments to identify patient needs and develop personalized care plans
  • Manage patients through acute medical situations, chronic conditions, and short-term rehabilitation episodes
  • Collaborate with interdisciplinary teams to ensure accurate and complete documentation and effective care coordination
  • Provide advanced illness and advanced care planning conversations, counseling, and education to patients and caregivers
  • Facilitate agreement and implementation of patient plans of care, engaging facility staff, families, and primary and specialty care clinicians

Requirements:
  • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program (for Nurse Practitioners) or accredited Physician Assistant degree program (for Physician Assistants)
  • Active and unrestricted license in the state of practice or ability to obtain by start date
  • Board certification through the American Academy of Nurse Practitioners or American Nurses Credentialing Center (for Nurse Practitioners) or National Commission on Certification of Physician Assistants (for Physician Assistants)
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating stairs and dwelling conditions
