We are looking for a dedicated Patient Support Coordinator to collaborate with our clinical team in fostering life-skills enhancement and promoting sustained recovery for individuals facing Substance Use Disorders (SUD) and Mental Health challenges.
Qualifications:
- Minimum of 2 years of experience in the behavioral health sector is preferred.
- Bachelor's degree is required.
- Certifications such as CDCA or QMHS are advantageous.
Key Responsibilities:
Your role commences upon a client's admission to our facility.
- Conduct an initial case management assessment with the patient to identify potential needs for Family Medical Leave Act (FMLA), Short-Term Disability Insurance, and any ongoing legal matters. Additionally, secure a Release of Information (ROI) for family members, Employee Assistance Programs (EAP), and/or employers. Ensure the patient has informed their family of their arrival.
- Prepare all necessary documentation for the patient's employers, educational institutions, legal entities, etc., including but not limited to FMLA, Short-Term Disability (STD), Long-Term Disability (LTD), and Treatment Letters.
- Schedule and conduct weekly case management sessions with each patient.
- Establish and maintain a strong, ongoing rapport with patients and their families, ensuring healthy communication.
- Formulate effective aftercare strategies for all patients, ensuring adherence to the established plans. Collaborate with the outreach team or designated company representative to develop suitable aftercare solutions.
- Regularly update and adjust all patient discharge dates as determined by the Utilization Review, Clinical, and Medical teams.
- Accurately convey information during team meetings.
- Ensure the transfer of medical records to the facility where the patient will be discharged.
- Facilitate assigned SUD and Mental Health group sessions.
- Maintain a fundamental understanding of health insurance benefits, terminology, and various payment methods utilized by our organization.
- Remind patients of program rules and regulations as necessary and appropriately report any infractions.
- Engage with patients respectfully while maintaining professional boundaries.
- Safeguard the confidentiality of all patient information in accordance with the organization's privacy policies and relevant federal and state laws, upholding the principles of professionalism as a healthcare provider.
