As a key member of our PACT program, you will play a vital role in delivering high-quality, patient-centered care to individuals with substance use and mental health conditions.
Key Responsibilities- Conduct comprehensive psychiatric assessments and develop individualized treatment plans.
- Provide medication management and medical services, including coordination with community physicians.
- Lead patient-centered comprehensive assessments, including mental status, physical health, and social development.
- Document patient progress and participate in staff meetings and individual service planning review meetings.
- Collaborate with multidisciplinary teams to ensure seamless care coordination.
- Develop and implement symptom education programs to empower patients to manage their mental health symptoms.
- Provide direct clinical services, including individual, group, and family therapy, to promote personal growth and development.
- Assist patients in accessing primary care physicians, dentists, and other medical specialists as needed.
- Support patients in achieving their goals and promoting independence.
- Master's degree in psychiatric mental health nursing.
- Licensed as a Washington State Advanced Registered Nurse Practitioner.
- Carry American Nurses Credentialing Center (ANCC) certification.
- At least one year of work experience in an inpatient or outpatient psychiatric setting.
- Ability to communicate effectively with patients, families, and healthcare teams.
- Strong organizational and time management skills.
- Ability to work in a fast-paced environment and prioritize multiple tasks.
- Commitment to delivering high-quality, patient-centered care.
