Transform Healthcare with Optum
We're seeking a dedicated and compassionate Nurse Practitioner or Physician Assistant to join our team in Rockingham and Hillsboro Counties, NH. As a Home-Based Medical Care Provider, you'll play a vital role in delivering high-quality medical care to patients in their homes.
About the Role
You'll work closely with our interdisciplinary team to develop and implement personalized care plans for patients. Your primary responsibilities will include:
- Conducting comprehensive assessments of patients and providing ongoing care;
- Developing and updating patient management care plans as needed;
- Making recommendations for diagnostic tests and interventions to prevent unnecessary hospitalizations;
- Educating patients on disease processes and self-care strategies;
- Participating in quality improvement initiatives to enhance patient outcomes.

- A competitive salary range of $120,000 - $160,000 per year, based on experience;

Requirements
To be successful in this role, you'll need:
- A completion of a Nurse Practitioner or Physician Assistant accredited program;
- A current state RN/NP or PA license;
- Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC) or Physician Assistants (NCCPA);
- Access to reliable transportation;
- Strong communication and interpersonal skills;
- A commitment to delivering high-quality patient care.
