Pediatric Home Care Nurse

We are seeking a compassionate and skilled Pediatric Private Duty LPN to join our team at Phoenix Home Care and Hospice. In this rewarding role, you will have the opportunity to make a meaningful difference in the lives of children and their families.
About the Job
- This is a full-time position with flexible scheduling options available, including days, evenings, nights, and weekends.
- You will be working with clients of various ages, from birth to 30 years old, who may require specialized care due to technologies such as g-tubes, trachs, and ventilators.
- New hire bonus of up to $2,500 available for overnight and weekend shifts.
Responsibilities
- Assist in developing, reviewing, and revising plans of care to ensure the highest quality of service.
- Provide nursing care in accordance with physician orders, individual plans of care, and payer source authorizations.
- Observe, record, and report any changes in client condition to appropriate personnel and/or physicians.
- Provide ongoing education and supervision to clients, families, or other healthcare team members on treatment plans.
- Monitor and document patient condition, provided care services via electronic charting.
- Participate in education programs, quality assessment activities, nurse meetings, and staff meetings as required.
Requirements
- Active Licensed Practical Nurse (LPN) or Registered Nurse (RN) license.
- Problem-solving skills and ability to multitask.
- Excellent communication skills.
- Positive, professional attitude.
