We are seeking dedicated individuals to join our innovative team. Our mission is to ensure that all individuals receive compassionate and high-quality mental health services, particularly those who have historically faced barriers to care. As we expand our services, we invite a Client Support Specialist to collaborate with our multidisciplinary team, which includes nurses, case managers, and therapists, all within our integrated agency. This is a remarkable opportunity to contribute to our vision of enhancing mental health support.

KEY DUTIES:

  1. Serve as a key communicator between the care team and clients' families.
  2. Facilitate family interactions, coordinate transportation, and address clients' needs effectively.
  3. Ensure consistent communication among families, referral sources, and PATH Integrated Health.
  4. Engage in a collaborative case management approach to support clients' overall health and wellness.
  5. Evaluate clients' unique needs and develop tailored care strategies.
  6. Work in conjunction with other team members to provide seamless care and support, enhancing clients' overall well-being.
  7. Regularly monitor clients' progress and adapt care plans to meet changing needs.
  8. Advocate for clients to access various resources, empowering them to navigate challenges and achieve their objectives.

REQUIREMENTS:

Education: A High School Diploma or GED is required; a Bachelor's degree is preferred.

Experience: A minimum of 1 year of experience in case management is required.

Physical Demands: The role requires the ability to work under pressure and manage irregular hours.

Communication Skills: Candidates must demonstrate excellent verbal and written communication abilities to interact professionally with clients and colleagues. Strong grammar and writing skills are essential for effective communication.

Interpersonal Skills: The ideal candidate will exhibit a friendly, compassionate, and professional demeanor when interacting with clients, families, and team members.

Technical Skills: Proficiency in Electronic Health Record (EHR) systems, telehealth services, and other relevant software is necessary for efficient client information management.

Background Check: Candidates must successfully pass all required federal and state background checks, and their educational and professional history will be verified in accordance with company policies.
