Our mission is to ensure that all individuals receive compassionate and high-quality mental health services, particularly those who have historically faced barriers to care. As we expand our services, we invite a Client Support Specialist to collaborate with our multidisciplinary team, which includes nurses, case managers, and therapists, all within our integrated agency. This is a remarkable opportunity to contribute to our vision of enhancing mental health support.
KEY DUTIES:
- Serve as a key communicator between the care team and clients' families.
- Facilitate family interactions, coordinate transportation, and address clients' needs effectively.
- Ensure consistent communication among families, referral sources, and PATH Integrated Health.
- Engage in a collaborative case management approach to support clients' overall health and wellness.
- Evaluate clients' unique needs and develop tailored care strategies.
- Work in conjunction with other team members to provide seamless care and support, enhancing clients' overall well-being.
- Regularly monitor clients' progress and adapt care plans to meet changing needs.
- Advocate for clients to access various resources, empowering them to navigate challenges and achieve their objectives.
REQUIREMENTS:
Education: A High School Diploma or GED is required; a Bachelor's degree is preferred.
Experience: A minimum of 1 year of experience in case management is required.
Physical Demands: The role requires the ability to work under pressure and manage irregular hours.
Communication Skills: Candidates must demonstrate excellent verbal and written communication abilities to interact professionally with clients and colleagues. Strong grammar and writing skills are essential for effective communication.
Interpersonal Skills: The ideal candidate will exhibit a friendly, compassionate, and professional demeanor when interacting with clients, families, and team members.
Technical Skills: Proficiency in Electronic Health Record (EHR) systems, telehealth services, and other relevant software is necessary for efficient client information management.
Background Check: Candidates must successfully pass all required federal and state background checks, and their educational and professional history will be verified in accordance with company policies.
