Job Title: Home Health Physical Therapist

We are seeking a skilled and compassionate Physical Therapist to join our Home Health team in Chestnut Hill, Massachusetts. As a Physical Therapist with LHC Group, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

About the Role:
  • Evaluate and assess patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions.
  • Develop and implement treatment plans to improve and enhance patient well-being.
  • Collaborate with the healthcare team to ensure comprehensive care and effective communication.
  • Provide education and support to patients and their families to promote independence and self-care.
Requirements:
  • Current Physical Therapy licensure in the state of Massachusetts.
  • Current CPR certification.
  • Must have a current driver's license and vehicle insurance, and access to a dependable transportation.
