Care Management LVN/LPN Full Time Position

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at CenterWell Home Health. As a Home Health LPN, you will play a vital role in providing high-quality care to patients in their homes.
Job SummaryThis is a full-time position that requires a strong commitment to delivering exceptional patient care. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
Responsibilities- Provide professional nursing services under the direction of a RN, adhering to state and federal regulations, as well as CenterWell Home Health policies and procedures.
- Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing targeted interventions.
- Collaborate with patients, families, and healthcare teams to establish and achieve patient-centered goals.
- Document patient progress, including visit notes, treatment plans, and outcomes, in a timely and accurate manner.
- Participate in ongoing education and training to stay current with best practices and industry developments.
- Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
- Current nursing license in the practicing state.
- Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
