Cardinal Health is seeking a dedicated pharmacist to oversee a 340B community pharmacy situated within a partner health center. Our pharmacists are crucial in providing medication and pharmaceutical care to underserved communities. This role encompasses immunizations, patient consultations, medication adherence monitoring, medication therapy management, and the oversight of high-risk patients.

Pharmacy Hours: Monday-Friday 8:30am-6:00pm

Key Responsibilities:

  • Collaborate with administrative and physician leadership within partner Community Health Centers to implement pharmacist-driven clinical and dispensing services.
  • Develop and execute collaborative practice agreements aimed at enhancing patient medication compliance for high-risk individuals and improving provider satisfaction.
  • Ensure adherence to best practices and compliance with 340B regulations in collaboration with relevant stakeholders.
  • Maintain adequate staffing levels to meet the operational needs of the pharmacy.
  • Keep precise records of all prescription medications received and dispensed.
  • Establish and enforce policies for accurate dispensing and labeling of prescriptions.
  • Safeguard the prescription area and its contents, ensuring unauthorized individuals are restricted from access during the pharmacist's temporary absence.
  • Supervise that only licensed pharmacists and interns under direct supervision provide professional consultations.
  • Oversee the pharmacy's Continuous Quality Improvement Plan to reduce medication error rates and enhance patient safety.
  • Prevent the dispensing or presence of misbranded, deteriorated, or outdated drugs in the pharmacy.
  • Maintain an orderly and sanitary prescription area.
  • Function as the Pharmacist in Charge in accordance with applicable pharmacy laws.
  • Ensure compliance with all Federal and State regulations governing pharmacy practice.
  • Perform additional duties as assigned.

Qualifications:

  • Bachelor's degree in pharmacy required; PharmD preferred.
  • Bilingual proficiency (Spanish, English) preferred.
  • Minimum of 3 years of pharmacy management experience; 4-5 years of overall pharmacy experience.
  • Active Wisconsin Pharmacist license in good standing.
  • Immunization Certification or willingness to obtain certification within 90 days of hire.
  • Ability to work a flexible schedule.
  • Strong interpersonal and customer service skills.
  • Effective multitasking and time management abilities.
  • Experience with 340B programs preferred.

Anticipated Salary Range: $108,300 - $162,395
