Job Summary:

As an Independent Optometrist at Sam's Club, you will have the autonomy to establish your days of practice, hours of operation, patient schedule, and fee schedule. You will be responsible for taking a leading role in patient care with respect to eye and vision care as well as general health and well-being.

Key Responsibilities:

  • Provide high-quality eye and vision care services to patients
  • Establish and maintain a patient schedule that meets your needs and goals
  • Set your own fee schedule and keep 100% of exam/professional fees
  • Have the option to opt into equipment/instrumentation and administrative/Opt-Tech support services for a fair market value

Requirements:

  • Doctor of Optometry (OD) from an accredited optometry school
  • Valid state optometry license
  • Ophthalmologist (MD or DO) from an accredited school
  • Valid state ophthalmology license
  • General Liability Certificate
  • Professional Liability Insurance Certificate
  • W-9
