We are seeking a skilled Licensed Practical Nurse to join our team at St Croix Hospice. As a Licensed Practical Nurse, you will play a vital role in providing direct patient care under the supervision of a registered nurse.
Responsibilities:- Follow the plan of care and provide treatments as defined in the State Nurse Practice Act.
- Work collaboratively with the healthcare team to achieve positive patient outcomes.
- Implement current nursing practice following a comprehensive assessment and the plan of care.
- Provide accurate and timely documentation of patient services to reflect the plan of care.
- Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care.
- Participate in coordination of hospice services and report identified needs to the interdisciplinary group.
- Use and prepare equipment and supplies effectively, adhering to aseptic technique.
- Provide appropriate pain/symptom management and evaluate patient's response to treatments/medications.
- Participate in personal, professional growth and development, and organization's quality assessment performance improvement teams and activities.
- Graduate of an accredited practical nurse or vocational nursing program.
- One to three years' nursing experience preferred, with community health/hospice or medical/surgical experience desired.
- Currently licensed as an LPN/LVN in the State.
- Complies with accepted professional standards and practice.
- Demonstrates good verbal and written communication, and organization skills.
- Possesses and maintains current CPR certification.
