Job Summary:

We are looking for a dedicated and compassionate Nurse Practitioner to provide high-quality care to our patients in a long-term care setting. The ideal candidate will have experience in geriatrics and be able to work independently with minimal supervision.

Responsibilities:

  • Provide primary and specialty care to patients in a long-term care setting
  • Conduct physical exams, take medical histories, and order diagnostic tests as needed
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Collaborate with other healthcare professionals to ensure comprehensive care
  • Document patient information and maintain accurate records

Requirements:

  • Active and unrestricted license in the state in which you reside, or ability to obtain by start date
  • Education: NP: Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
  • NP: Board certified through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center, with certification in one of the following: Family Nurse Practitioner Adult Nurse Practitioner Gerontology Nurse Practitioner Adult-Gerontology Acute Care Nurse Practitioner
  • Ability to complete physical requirements of the job including lifting a 30-pound bag in and out of the car, navigating stairs and a variety of dwelling conditions, and sit, stand, and kneel to perform physical assessment
  • Access to reliable transportation
  • Ability to gain a collaborative practice agreement, if applicable in your state
  • Full COVID-19 vaccination is an essential requirement of this role
