Nurse Practitioner/Physician Assistant

Our Senior Community Care (SCC) product is a pioneering effort to provide holistic care to patients in their homes, nursing homes, and assisted living facilities. As a Nurse Practitioner/Physician Assistant, you'll be part of a team that's changing the face of healthcare.
Key Responsibilities:- Deliver high-quality, cost-effective care to assigned members
- Manage medical and behavioral conditions, collaborating with physicians and specialty providers
- Perform comprehensive assessments, documenting findings in a concise and compliant manner
- Ensure accurate diagnoses, ICD10 coding, and documentation
- Coordinate care, facilitating agreement and implementation of the member's plan of care
- Monitor the effectiveness and efficiency of the plan, making revisions as needed
- Certified Nurse Practitioner or Physician Assistant through a national board
- Graduate of an accredited master's degree in Nursing or Physician Assistant degree program
- Active and unrestricted license in the state of practice
- Current DEA licensure/prescriptive authority or ability to obtain post-hire
- Access to reliable transportation and ability to lift 30 pounds
- Availability to work 24 hours per month, with 16 hours during off-hours
- 1+ years of hands-on post-grad experience in Long Term Care
- Understanding of Geriatrics and Chronic Illness
- Understanding of Advanced Illness and end-of-life discussions
- Proficient computer skills and ability to document medical information
