Palliative Care Social Worker Job Description

The Palliative Care Social Worker plays a vital role in providing comprehensive care to patients and their families. This position requires a strong understanding of palliative care principles and the ability to work collaboratively with the Palliative Care Team and other healthcare professionals.

Key Responsibilities:
  • Conduct psychosocial assessments to identify patient and family needs
  • Develop and implement treatment plans to address patient and family needs
  • Provide emotional support and counseling to patients and families
  • Collaborate with the Palliative Care Team to provide coordinated care
  • Communicate effectively with patients, families, and healthcare professionals
Requirements:
  • Masters Degree in Social Work from an accredited school
  • 2 years of experience in palliative and hospice care
  • 2 years of experience as a medical social worker
  • SW-LIC - PA Social Worker License (preferred)
  • CPR - Cardiopulmonary Resuscitation (preferred)
