Job Title: Clinical Coordinator

North Suffolk Community Services Inc. is seeking a skilled Clinical Coordinator to join our Intensive Community Wrap-Around Services Team.

Job Summary:

The Clinical Coordinator will lead the team in assessment, treatment planning, and service delivery processes, providing coaching and support to individuals in home and community settings. This role requires a strong understanding of Positive Behavior Supports and Recovery tenets, as well as experience working with adults with chronic and persistent mental health issues and/or substance use disorders.

Key Responsibilities:
  • Engage individuals in their recovery journey, promoting overall health and well-being.
  • Assist individuals with autism spectrum disorder, psychiatric, and substance use disorders to identify personal priorities and establish goals that support a life in recovery.
  • Provide clinical services in a timely and flexible manner, consistent with trauma-informed care and person-centered approaches.
  • Carry out assigned interventions and action steps of the treatment plan, providing direct support and rehabilitation services to individuals in individual, group, and family settings.
  • Organize and lead individual and group social and recreational activities to structure each person's time, increase social experiences, and provide opportunities to practice social skills.
  • Document each person's progress in the confidential record according to established methods and procedures.
Requirements:
  • LPHA licensure required: LICSW, LCSW, LMHC, LRC, LADC1, M.D., R.N., Registered Occupational Therapist, or licensed Psychologist.
  • Previous experience in the provision of clinical or rehabilitative services to adults with chronic and persistent mental health issues and/or substance use disorders preferred.
  • Previous supervisory experience preferred, but not required.
