Job Title: Speech Language Pathologist - Home Health

Supplemental Health Care is seeking a skilled Speech Language Pathologist to join our team in Jacksonville, Florida. As a Speech Language Pathologist, you will work with patients in their homes, providing personalized care and support to help them achieve their goals.

Job Summary:
  • Provide speech-language pathology services to patients in their homes, including assessment, diagnosis, and treatment of communication and swallowing disorders.
  • Develop and implement individualized treatment plans to meet the unique needs of each patient.
  • Collaborate with other healthcare professionals to ensure comprehensive care and support for patients.
  • Document patient progress and maintain accurate records of treatment plans and outcomes.
Requirements:
  • Graduate of an accredited SLP program.
  • Licensed as a Speech Language Pathologist in Florida.
  • CPR Certified or ability to obtain.
  • Previous experience with Computer Charting.
  • 6 months of previous Home Health Care experience.
  • Valid U.S. Driver's License and reliable transportation.
