Public Partnerships | PPL is seeking a highly motivated and results-driven Support Broker to join our team. As a Support Broker, you will play a critical role in supporting individuals with disabilities or chronic illnesses and aging adults to remain in their homes and communities.
Key Responsibilities- Provide information and education to Medicaid members who participate in self-direction programs, as well as their authorized representatives.
- Evaluate the participant's home environment, noting and attempting to resolve any safety concerns through our mandated escalations process.
- Work with participants to develop and implement personalized care plans that meet their unique needs and goals.
- Collaborate with healthcare professionals, family members, and other stakeholders to ensure seamless care coordination.
- Manage a caseload of participants, prioritizing tasks and ensuring timely completion of in-home visits and other responsibilities.
- Bachelor's degree in social work, case management, or a related field.
- At least one year of experience working with vulnerable populations in their homes.
- Strong working knowledge of Microsoft Office products, including Outlook.
- Familiarity with CRM tools and electronic medical records.
- Valid driver's license and satisfactory driving record.
- Reliable automobile and secure, robust, and reliable wireless service.
