Job Overview:

We are seeking a skilled Healthcare Coordinator Specialist to join our team at Compunnel. In this role, you will provide comprehensive case management and care coordination for a diverse panel of patients with identified needs spanning age, condition, and population variables.

Estimated Salary: $120,000 - $160,000 per annum, depending on experience.

Key Responsibilities:

* Collaborate with healthcare providers, nurses, social workers, patients, families, and caregivers to ensure comprehensive care and optimal patient outcomes.

* Develop and implement personalized care plans that address the unique needs of each patient.

* Provide coaching and education to patients to improve their health behavior and attain health-related goals.

* Recognize and reward performance in a challenging environment.

Required Qualifications:

* Graduate from an accredited school of nursing.

* Active, unrestricted licensure as a Registered Nurse in a state.

* Current state driver's license with the ability to travel to hospitals, skilled nursing facilities, patients' homes, and other care sites as needed.

* Full COVID-19 vaccination is an essential job function; compliance with state booster requirements is necessary.

* Ability to perform all essential job functions with or without reasonable accommodation.

Preferred Qualifications:

* Certification in Geriatric Nursing or Geriatric Case Management.

* 5+ years of nursing experience, preferably with older patients.

* Proficient in computer use, the Internet, and health information technology.

* Demonstrated excellent communication, interpersonal, and organizational skills with a flexible approach to problem-solving.
