Job Title: Targeted Case Manager

We are seeking a highly motivated and compassionate Targeted Case Manager to join our team at Hire Health. As a key member of our team, you will play a vital role in assisting clients and families in accessing behavioral health treatment, medical interventions, educational, social, development, and other support services.

Job Summary:

The Targeted Case Manager is responsible for advocating for the needs of clients and families, working closely with our Outpatient clinical team and community partners to ensure the client's needs are being met across settings. You will focus on linking clients to local resources and enhancing social and emotional functioning and life skills.

Key Responsibilities:
  • Conduct comprehensive assessments and periodic reassessments to determine client needs for medical, social, educational, or other services.
  • Develop and implement individualized care plans with clients and families, with periodic revisions as needed.
  • Provide referral and related activities to help clients obtain needed services with medical providers, social providers, educational providers, or other programs and services.
  • Coordinate home visits, discharge planning, and provide monitoring or follow-up activities necessary to ensure the client's care plan is implemented and needs are being met.
  • Ensure client enrollment in educational programs, attend school meetings and Parent/Teacher conferences, and complete documentation accordingly.
  • Provide transportation for clients to various appointments, court, meetings, and home visits, as needed.
Requirements:
  • BS or BA in social work or a related behavioral science field, with a minimum of one year full-time experience working with individuals in a human service setting.
  • Master's degree in social work or a related behavioral health field may be substituted for the one year of experience.
  • Must successfully complete case management training within 6 months of hire and annual continuing education requirements.
  • Organizational skills and basic computer skills.
  • Must have a current driver's license and driver's insurance.
  • Demonstrates ability to work as an effective member of a multi-disciplined team.
  • Demonstrate proven sensitivity to the various cultural and socioeconomic characteristics of clients and staff.
Working Conditions:

The employee is regularly required to stand, walk, and talk or hear; use hands to finger, handle, or feel objects; and reach with hands and arms. The employee is occasionally required to sit, kneel, squat, and hold objects or carry. The employee must occasionally lift and/or move up to 10 pounds, and sometimes move and/or lift up to 100 pounds in order to utilize Safe Crisis Management techniques.

The noise level in the work environment is usually moderate. The environment consists of indoor and outdoor activities with different temperatures. Cleaning and using cleaning materials, including chemicals, are a demand of this position.
