Job Summary

We are seeking a skilled and compassionate HouseCalls Dual Statewide Traveler - Home Healthcare Specialist to join our dynamic team. This role involves conducting in-home assessments, identifying diagnoses, and communicating findings to inform member's primary care physician of potential gaps in care. You will work collaboratively with our team to provide holistic care that addresses patients' physical, mental, and social needs.

Responsibilities
  • Conduct in-home assessments, including evaluations of past medical history, reviews of systems, medication reconciliation, vital signs, and comprehensive physical exams
  • Perform evidence-based practice screenings, including point-of-care testing (as appropriate)
  • Identify diagnoses to be used in care management and active medical management
  • Formulate lists of current and past medical conditions using clinical knowledge and judgment
  • Communicate assessment findings to inform member's primary care physician of potential gaps in care
  • Educate members on topics such as disease processes, medications, and compliance
Requirements
  • Active NP or PA license or ability to obtain by start date
  • Active national NP or PA certification or ability to obtain national certification in state of assignment by start date
  • Ability to complete physical requirements of the job, including lifting 30-pound bags, navigating stairs, and sitting, standing, and kneeling to perform physical assessments
  • Access to reliable transportation for travel up to 85% within licensed states
Preferred Qualifications
  • 1+ years of clinical experience in highest level of education, clinical setting
  • Experience in Gerontology, Cardiology, Internal Medicine, or Endocrinology
  • Home healthcare or home visit experience
Compensation and Benefits

The estimated salary range for this role is $88,000 to $173,200 annually, depending on education, work experience, certifications, and other factors. In addition to your salary, UnitedHealth Group offers a comprehensive benefits package, incentive programs, equity stock purchase opportunities, and 401(k) contributions.
