We are seeking a skilled Licensed Practical Nurse (LPN) to join our team in providing high-quality home health care services to pediatric patients in the Palmer, MA area.

Job Summary

The successful candidate will be responsible for delivering and coordinating quality patient care in compliance with physician orders. This includes observing and assessing patient condition and care needs, documenting all activities and assessments, and participating in the development and implementation of the nursing care plan.

Responsibilities
  • Implement physicians' prescribed plan of care
  • Conduct patient assessments and coordinate care
  • Administer prescribed medication, treatments, and therapies
  • Document care provided in a timely manner
  • Provide education to patients and families on disease process, self-care techniques, and prevention strategies
  • Participate in educational programs and continued education units to maintain knowledge of competencies related to the nursing profession
Requirements
  • Graduate of an accredited school of nursing
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures
  • Minimum six months of recent experience as a Licensed Nurse in a clinical care setting
  • Home health experience preferred
  • Ability to speak, write, read, and understand English
  • Ability to travel and reposition patients and move equipment without assistance
