About the Role

This is a full-time position that involves working a 40-hour week. You will be responsible for assisting licensed nursing staff in providing care to patients with complex needs. Your duties will include utilizing glucose monitoring machines, making observations of patient conditions, documenting care provided, and responding to patient changes.

  • Utilize glucose monitoring machines to receive glucose readings for patients and refer deviations to appropriate healthcare staff.
  • Make observations of subtle changes in patient conditions, such as behavior shifts, mood, skin coloring, appetite, etc., and take action accordingly.
  • Document care given, observations, and responses on appropriate tools or forms.
  • Respond appropriately to subtle patient changes by contacting the appropriate nursing personnel.
  • Administer complete tracheostomy care, including suctioning.
  • Perform nursing procedures within scope of practice and according to policies and standard procedures.
  • Recognize residents' autonomy and promote independence through respect, limit setting, and individualized treatment.
  • Actively participate in treatment planning meetings.
  • Utilize equipment to promote veteran self-care, such as lift equipment, Hydro sound Tubs, and Veteran/Resident Wandering System.
  • Verbalize patient information to the treatment team for inclusion in the patient's plan of care and participate in treatment team meetings for resident care planning.
  • Enter information about the patient's condition in the chart, utilizing appropriate formats.
  • Participate in maintaining a clean, safe environment for patients.
  • Remove defective equipment, report hazards, observe safety regulations, contribute to infection control, and utilize proper aseptic technique in performance of duties.
  • Serve as the unit safe patient handling liaison.
  • Orient members of the health care team to the unit as appropriate.
  • Function as a member of the unit base counsel.
  • Manage distributed patient behavior and intervene appropriately, requesting assistance from other health care members when necessary.
  • Initiate basic life support and take appropriate action.
