Comunilife Inc. is seeking a dedicated Case Manager to join our Life is Precious team in Long Island, NY. As a Case Manager, you will play a vital role in supporting Latina teens and their families through our non-clinical suicide prevention afterschool program.

Key Responsibilities
  • Provide outreach, engagement, and follow-up services to ensure continuity of mental health treatment and encourage participation in the Life is Precious program.
  • Offer case management and counseling to connect clients with necessary services.
  • Develop and implement age-appropriate programming to expand horizons and provide healing experiences for clients.
  • Support vocational exploration, internships, and community service to enhance participants' career planning and personal growth.
  • Coordinate trips, celebrations, and special events to foster cultural pride and awareness.
  • Collaborate with community agencies, public officials, and organizations to promote program success.
  • Participate in program team meetings to discuss accomplishments and program-related matters.
  • Represent the program to community providers and provide information and training relevant to the program focus and populations.
  • Document community activities, outings, and development involving families.
