Case Manager Life Is Precious, Long Island

Comunilife Inc. is seeking a dedicated Case Manager to join its Life is Precious team in Long Island, NY. As a Case Manager, you will play a vital role in supporting Latina teens ages 12-17 and their families through a non-clinical suicide prevention afterschool alternative program.

Key Responsibilities:

  • Provide outreach, engagement, and follow-up with program participants to ensure continuity of mental health treatment and encourage participation in the Life is Precious program.
  • Offer case management and counseling services to connect clients with necessary resources.
  • Collaborate with the program team to develop age-appropriate activities that promote healing, growth, and cultural awareness.
  • Support vocational exploration, internships, and community service to enhance participants' career planning and personal growth.
  • Coordinate trips, celebrations, and special events to foster cultural pride and awareness.
  • Build relationships with community agencies, public officials, and organizations to ensure a comprehensive support network.
  • Participate in program team meetings to discuss accomplishments and program-related matters.
  • Represent the program to community providers and provide information and training relevant to the program focus and populations.

Requirements:

  • Bachelor's degree in human services or a related field, preferably in social work.
  • Experience working with adolescents and families, with a focus on Hispanic populations.
  • Bilingual skills in Spanish are required.

Preferred Qualifications:

  • Proficiency with MS Office and Google product suites.
  • Cultural competency in working with Hispanic adolescents and their immigrant families.
  • Proven ability to provide strong social service support and counseling.
  • Ability to apply analytical thinking to program matters.
