Physical Therapist for Subacute Care

We are seeking a skilled Physical Therapist to join our team at Doctors Subacute Care. As a Physical Therapist, you will work collaboratively with our therapy teams to provide high-quality care to our residents.
Responsibilities:- Conduct thorough evaluations and assessments of residents' physical abilities and create personalized treatment plans.
- Develop and implement effective therapy programs to improve residents' mobility, strength, and function.
- Work closely with our interdisciplinary team to ensure seamless care coordination and communication.
- Document residents' progress and maintain accurate records.
- Stay up-to-date with the latest best practices and industry developments in physical therapy.
- Current New Jersey Physical Therapy License.
- Experience working in long-term care settings, preferably in a Skilled Nursing Facility.
- Strong organizational, interpersonal, and communication skills.
- Ability to work independently and as part of a team.
