Physical Therapist for Subacute Care

We are seeking a skilled Physical Therapist to join our team at Doctors Subacute Care. As a Physical Therapist, you will play a vital role in helping our patients recover from illness or injury and regain their independence.
Responsibilities:- Work collaboratively with the therapy team to develop and implement individualized treatment plans
- Conduct thorough evaluations and assessments to identify patient needs and goals
- Provide hands-on therapy to patients, including exercises, stretches, and other interventions
- Document patient progress and communicate with healthcare team members to ensure seamless care
- Stay up-to-date with the latest research and best practices in physical therapy to provide evidence-based care
- Current New Jersey Physical Therapy License
- Experience working in a long-term care setting, preferably in a skilled nursing facility
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
