Nurse Practitioner

We are seeking a highly skilled Nurse Practitioner to join our Primary Care team at Gouverneur Hospital - Edwards Health Center. As a key member of our healthcare team, you will provide high-quality patient care and work collaboratively with our physicians and staff to deliver exceptional patient outcomes.
Responsibilities- Provide primary care services to patients, including health assessments, diagnosis, and treatment of acute and chronic conditions
- Develop and implement individualized care plans to meet the unique needs of each patient
- Collaborate with other healthcare professionals to ensure comprehensive and coordinated care
- Stay up-to-date with the latest medical research and best practices to provide evidence-based care
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions
- Master's degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) degree
- Current licensure as a Registered Nurse (RN) in New York State
- Nurse Practitioner certification in Primary Care
- Ability to work in a fast-paced environment and prioritize multiple tasks
- Excellent communication and interpersonal skills
- Competitive salary range: $100,000 - $200,000 per year
