We are seeking a highly skilled and compassionate RN to join our team as a Care Coordinator. As a Care Coordinator, you will play a vital role in coordinating care and providing ongoing nursing assessments for patients and their families. Your expertise will help ensure that patients receive the highest level of medical care and support.
Key Responsibilities:- Conduct home visits and telephonic case management to assess patient needs and coordinate care with interdisciplinary team members.
- Develop and implement individualized plans of care, recommending revisions as necessary.
- Provide education and support to patients and their families on disease management, pain and symptom control, and end-of-life care.
- Collaborate with healthcare providers to ensure seamless care coordination and communication.
- Document patient progress and outcomes in the electronic health record.
- Current RN licensure in the state of employment.
- 3+ years of experience in a physician's office, clinical, hospice, or hospital setting.
- Excellent nursing skills and ability to interact productively with individuals and multidisciplinary teams.
- Reliable transportation for daily travel to various locations as assigned.
- Hospice and Palliative Care Nursing certification.
- 2+ years of hospice experience.
- Experience with home visits and community-based nursing.
