About the Role:

Fairview Health Services is seeking a skilled RN Patient Care Management Coordinator to join our team. As a key member of our Inpatient Care Coordination Department, you will play a vital role in supporting patients and families as they navigate the healthcare system.

Key Responsibilities:

  • Develop and implement comprehensive care plans that address the unique needs of each patient.
  • Collaborate with interdisciplinary teams to ensure seamless transitions of care and optimal patient outcomes.
  • Provide education and support to patients and families to empower them to take an active role in their care.
  • Identify and address potential barriers to care, such as social determinants of health and access to community resources.
  • Document patient progress and outcomes in a timely and accurate manner.

Requirements:

  • Active RN license in Minnesota
  • 5 years of clinical experience
  • AD in Nursing

Preferred Qualifications:

  • BSN
  • 1+ years of experience in care coordination or case management
  • Basic Life Support Certification (BLS)
