We are seeking a highly skilled Registered Nurse Case Manager to join our team at Mass General Brigham. As a key member of our Patient Transfer and Access Center, you will play a critical role in managing capacity across our system, ensuring timely access to care for patients.
Key Responsibilities- Develop and implement strategies to optimize capacity and reduce bed-days across the system.
- Collaborate with hospital-based Case Managers, capacity leaders, and other stakeholders to identify and address barriers to discharge.
- Work closely with the Repatriation Program team to manage and coordinate repatriations, ensuring safe and efficient transfer of patients.
- Provide situational awareness and data-driven insights to inform capacity management decisions.
- Develop and maintain strong relationships with hospital-based Case Managers, capacity leaders, and other stakeholders to ensure seamless communication and collaboration.
- Graduate of an approved school of nursing, BSN required.
- Current license to practice as a registered nurse in the State of Massachusetts.
- Case Manager Certification preferred.
- 5+ years of Case Management experience at an acute care hospital.
- Extensive knowledge of case management and capacity management principles.
