Mass General Brigham is seeking a highly skilled Registered Nurse to join our Case Management team in Medford, Massachusetts.
Job Summary:The Registered Nurse - Case Management Specialist will play a critical role in supporting case managers in designated areas with complex patient discharges. This role will work closely with the Capacity Case Manager to ensure timely and appropriate transitions in care occur and alternate solutions and plans to discharge are thoroughly explored.
Key Responsibilities:- Act as a case manager expert and mentor to identify and resolve issues impacting quality, safety, and timeliness of care transitions.
- Co-manage with unit-based case managers complex patient care discharges and collaborate with relevant clinical teams both internally and externally.
- Identify potential discharge barrier mitigation strategies to ensure care progression and safe care transitions.
- Provide mentorship and guidance to clinical teams and case management as needed related to complex care and post-acute discharge.
- Monitor ongoing performance of initiatives and develop recommendations/identifies opportunities for performance improvement.
- RN with current license to practice in Massachusetts required.
- Bachelor's degree required; master's degree preferred.
- Case Manager certificate preferred.
- 5+ years of inpatient case management experience with advanced knowledge around standards of practice and regulatory and payer compliance required.
- Extensive experience and knowledge in discharge planning required.
- Extensive understanding of post-acute resources required.
