Mental Health Practitioner Job Description

RADIAS Health is seeking a skilled Mental Health Practitioner to join our Community Care Service Team (CCST). This role specializes in supporting individuals with high unmet needs and high utilizers of emergency room/hospital services.

Key Responsibilities:
  • Develop and implement treatment plans in collaboration with clients and the treatment team.
  • Assist clients in establishing community supports and communicating with natural supports.
  • Coordinate services with other providers, including local community hospitals, mental health case management, and outpatient behavioral health providers.
  • Develop individualized crisis plans and meet with clients to explain CCTS services, assess client needs, and establish a care plan.
  • Assist clients with psychiatric rehabilitation, independent living skills, and obtaining/maintaining housing.
  • Attend commitment and court hearings as needed and complete required court reports.
Requirements:
  • Bachelor's degree in behavioral sciences or related field (or 30 semester credit hours in behavioral health sciences) and 2,000 hours supervised work experience with individuals diagnosed with serious mental illness and/or substance use disorders.
  • OR Master's or other graduate degree in behavioral health sciences or related field.
  • OR 4,000 hours supervised work experience with individuals diagnosed with serious mental illness and/or substance use disorders.
