Thomas Jefferson University Hospital is seeking a highly skilled and compassionate Ambulatory Nurse Specialist to join our team. As a key member of our primary care clinical team, you will provide exceptional patient care, collaborate with healthcare providers, and contribute to the development of clinical care pathways.
Responsibilities- Provide Patient-Centered Care: Utilize evidence-based practice, clinical expertise, and patient preferences to deliver optimal healthcare.
- Collaborate with Healthcare Providers: Communicate effectively with primary care providers, specialists, and support staff to ensure seamless patient care.
- Develop Clinical Care Pathways: Participate in the implementation of clinical care pathways for common primary care conditions and contribute to quality improvement initiatives.
- Provide Education and Support: Educate patients and their families on preventive care, chronic disease management, and medication management.
- Document Patient Interactions: Accurately document patient interactions and clinical care in the electronic medical record.
- Perform Care Coordination: Coordinate patient care, including abnormal results, triage, and RN verification of vaccinations and other injections.
- Oversee Clinical Care: Ensure the assessment, promotion, maintenance, and restoration of patients' health and wellness.
- Minimize Risk: Follow established nursing practices and protocols to minimize risk to patients and their families.
- Utilize Technology: Leverage information technology to communicate, manage knowledge, and mitigate errors.
- Monitor Outcomes: Use data to monitor the outcomes of care processes and support ethical decision-making.
- Function within Inter-Professional Teams: Collaborate effectively with primary care inter-professional teams to achieve quality patient care.
- Education: Graduate of an accredited school of nursing required; BSN or completion of BSN within 3 years of hire strongly preferred.
- Experience: 2+ years' experience as a Nurse, in primary care or ambulatory setting preferred.
- Certifications: Current RN licensure in state of employment; AHA BLS certification.
