We are seeking a skilled and compassionate Registered Nurse to join our Home Health team at the University of Pittsburgh Medical Center. Key Responsibilities- Coordinate case management activities within an interdisciplinary healthcare team to ensure appropriate delivery of services to patients.
- Attend compliance training and adhere to organizational standards of conduct, policies, and procedures.
- Focus on customer service and continually strive to perform duties in a manner that results in optimal patient satisfaction.
- Complete initial and comprehensive assessments of patients.
- Constructively receive feedback and direction.
- Take action to improve knowledge, skills, and performance based on feedback or self-identified developmental needs.
- Initiate, coordinate, and ensure ongoing communication with the physician and other members of the interdisciplinary healthcare team, including evaluating patients and family-centered health teaching.
- Provide direct patient care, evaluate outcomes, and adjust nursing care processes as indicated to ensure optimal patient care.
- Establish the plan of care in collaboration with the physician, patient, and interdisciplinary team members in accordance with physician orders and patient needs.
- Assign other interdisciplinary team members as appropriate to assist in the delivery of services as ordered on the plan of care to the patient.
- Supervise and teach other nursing personnel, including LPNs and Home Health Aides, as needed.
- Attend and participate in case conferences, team meetings, and other work-related meetings.
- Recognize limitations and request assistance and direction to meet performance goals.
- Complete all necessary documentation, including visit assessment, plan of treatment, and verbal orders, accurately and promptly in the electronic documentation system while in the patient's home and within the agency and regulatory standards.
- Data syncs (transmits) information same day.
- Identify, investigate, and recommend solutions to organizational issues.
- Exercise independent judgment in matters concerning emergent and non-emergent patient care needs and communicate with the physician.
- Identify, develop, and participate in process improvement opportunities within the agency that will enhance the quality of the services provided.
- Participate in weekend and on-call coverage rotation for the agency as per BU guidelines.
- Demonstrate knowledge and understanding of the UPMC standard of care delivery and proficiency in all aspects of patient care, to include complex and/or specialized care.
- Zero to two years of experience preferred.
- Six months of nursing experience in an Acute Care or Skilled Care Facility or Home Health or Hospice Care preferred.
- BSN preferred.
- Ability to establish and maintain positive, caring relationships with executives, managers, physicians, non-physician providers, ancillary and support staff, other departments, and patients/families.
- Ability to work productively and effectively within a complex environment, handle multiple/changing priorities, and specialized equipment.
- Good clinical judgment with critical thinking, analytical, and problem-solving abilities required as related to various aspects of patient care.
- Critical thinking skills necessary to exercise and to lead others in application of the nursing process.
- Mobility and visual manual dexterity.
- Physical stamina for frequent walking, standing, lifting, and positioning of patients.
- Successful completion of Home Health/Hospice Orientation upon hire.
- For a MENTAL HEALTH NURSE, these additional items are required:
- One year of nursing experience required.
- Previous experience in a mental health setting required.
- Must successfully complete a structured mental health training program upon hire.
- Current licensure as a Registered Professional Nurse either in the state where the facility is located or in a state covered by a licensure compact agreement with the state where the facility is located.
- CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire.
- Graduate nurses must complete licensure examination within SIX (6) MONTHS of hire or within one year of graduation, whichever comes first.
