Key Responsibilities:- Conduct comprehensive patient assessments to evaluate speech, language, voice, and swallowing disorders
- Develop and implement individualized treatment plans based on patient needs and goals
- Provide evidence-based therapy sessions to improve communication skills and swallowing abilities
- Collaborate with healthcare professionals to ensure comprehensive patient care and coordination
- Monitor patient progress and adjust treatment plans as necessary to achieve optimal outcomes
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care
- Maintain accurate and detailed documentation of patient evaluations, progress notes, and treatment outcomes
- Master's degree in Speech-Language Pathology or related field
- Current licensure as a Speech Language Pathologist (SLP) or Certified Fellow (CF)
- Experience in skilled nursing, long-term care, or acute care settings
- Knowledge of anatomy related to speech, language, voice, and swallowing functions
- Strong case management skills to coordinate care with other healthcare professionals
- Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare teams
