Job Summary:We're seeking a skilled Care Manager - Member Advocacy to develop, assess, and coordinate care management activities based on member needs to provide quality, cost-effective healthcare outcomes. This role requires 50% field-based work.
Key Responsibilities:- Evaluate member needs, barriers to care, and resources available to recommend and facilitate the best outcome.
- Develop personalized care plans and work with providers, specialists, and community resources to ensure timely access to care.
- Provide psychosocial and resource support to members, caregivers, and care managers to access local resources or services.
- Coordinate between members, caregivers, and care providers to ensure identified care or services are accessible in a timely manner.
- Monitor progress towards care plan goals and collaborate with healthcare providers for plan revision or address identified member needs.
- Collect, document, and maintain all member information and care management activities to ensure compliance with current state, federal, and third-party payer regulators.
- Perform on-site visits to assess member needs and collaborate with providers or resources as needed.
- Provide education to care managers and members on procedures, healthcare provider instructions, care options, referrals, and healthcare benefits.
- Bachelor's degree and 2-4 years of related experience.
- Current state's clinical license preferred.
