Dynamic Physical Rehabilitation Specialist

JAG Physical Therapy is seeking a skilled Physical Therapist to join our team in Marine Park. This role involves assessing patients, creating individualized treatment plans, and providing effective rehabilitation in a supportive environment.
About JAG Physical TherapyWe are a comprehensive outpatient orthopedic physical therapy company with 100 facilities throughout Pennsylvania, New Jersey, and New York. Our care-first model of rehabilitation focuses on patient confidentiality and a positive atmosphere, allowing our therapists to collaborate with other healthcare professionals to maximize patient recovery and independence.
Responsibilities- Collects and records patient care data that follows format and guidelines of the patient care service, regulatory agencies, and third-party payers which reflects an understanding of reimbursement systems and their impact on patient care.
- This includes initial evaluation, daily notes, reassessments/progress reports, discharge plans, and communication with members of the team.
- Daily billing and following department guidelines for productivity.
- Evaluation preparation according to department policies/procedures and standards of practice, including obtaining patient history, exhibiting knowledge of patient diagnoses, and researching unfamiliar diagnoses.
- Selecting and administering tests and measures.
- Synthesizing evaluation results to formulate diagnosis, goals, prognosis, and plan of care.
- Establishing treatment plan based on evaluation results, goals, and best practice.
- Collaborating with interdisciplinary team, patients, and patient's support system.
- Bachelor's or Master's Degree required, Doctorate degree preferred.
- Licensed Physical Therapist in the state practicing in.
- Completed state regulated CEU requirement in appropriate state (NY, NJ).
- Valid CPR License.
- Experience/knowledge with EMR software is preferred.
- Excellent oral, written communication.
- Strong clinical decision-making skills.
- Excellent work ethic and dedication to patient success.
- Basic computer skills.
- Works as a team member and individually with minimal supervision.
- Maintains a positive, professional demeanor at all times.
