Job SummaryWe are seeking a skilled Licensed Practical Nurse (LPN) to join our team in a hospital setting. As a Behavioral Health LPN, you will play a critical role in providing compassionate and individualized care to our patients.
Responsibilities- Participate in the planning, coordination, and documentation of total patient care in conjunction with the multidisciplinary treatment team.
- Accurately report and record the patient's condition and care, including signs and symptoms that may be indicative of change.
- Prepare clinical/progress notes daily and incorporate them into the clinical record weekly, and document monthly recertification/Master Treatment Plan Updates in coordination with the multidisciplinary treatment team.
- Initiate and sustain the implementation of all physician orders.
- Complete, maintain, and submit accurate and relevant clinical notes.
- Participate in case conferences, in-services, and other programs and meetings as needed.
- Facilitate nursing psych-education groups as needed, ensuring complete and timely documentation of each group facilitated.
- Complete Nursing Health Screens for every new admission and communicate any issues or concerns with the physician.
- Participate in the development and implementation of the Plan of Care in coordination with other members of the multidisciplinary treatment team.
- Perform other duties and special projects as required.
- Graduate of an accredited school of nursing.
- Must have Practical Nurse Licensure in the state of operation.
- Minimum of 1 year experience as a Licensed Practical Nurse, with experience in behavioral health care preferred.
- Certification in Basic Life Support required. Will provide at company's cost if needed.
- Certification in non-violent crisis intervention. Will provide at company's cost if needed.
