Job Title: Public Health Nurse II

Job Summary:

The County of San Bernardino, CA is seeking a highly skilled Public Health Nurse II to join our team. As a Public Health Nurse II, you will be responsible for providing assessments of individuals, families, and communities, as well as developing treatment plans and providing consultation on medical, dental, and behavioral health assessments.

Key Responsibilities:

  • Provide assessments of individuals, families, and communities
  • Develop treatment plans
  • Provide consultation on medical, dental, and behavioral health assessments
  • Act as a nurse liaison with public and private agencies
  • Provide assessments and referral for school district students
  • Conduct disease control measures

Requirements:

  • Valid Registered Nurse (RN) license issued by the California Board of Registered Nursing (CA BRN)
  • Valid Public Health Nurse (PHN) certificate issued by the CA BRN
  • Valid Healthcare Provider Basic Life Support (BLS) certificate
  • Three (3) months of full-time equivalent experience as a licensed Public Health Nurse or three (3) months of full-time equivalent experience in home health or home visiting as a licensed Registered Nurse or licensed Public Health Nurse

Preferred Qualifications:

  • Experience working with pediatric populations
  • Experience using computer case management/database programs
  • Experience working with a California Children's Services (CCS) agency

Working Conditions:

Travel throughout the county is required. Employees will be required to make provision for transportation. A valid California Class C driver license and proof of automobile liability insurance must be produced for the individual providing the transportation.
