Job Description

Job Title: Public Health Nurse II

Job Summary: We are seeking a highly skilled Public Health Nurse II to join our team. The successful candidate will be responsible for providing assessments of individuals, families, and communities, as well as coordinating and assisting with access to care, developing treatment plans, and providing consultation on medical, dental, and behavioral health assessments.

Responsibilities:

  • Provide assessments of individuals, families, and communities
  • Coordinate and assist with access to care
  • Develop treatment plans
  • Provide consultation on medical, dental, and behavioral health assessments
  • Act as a nurse liaison with public and private agencies
  • Provide assessments and referral for school district students
  • Conduct disease control measures

Requirements:

  • Valid Registered Nurse (RN) license issued by the California Board of Registered Nursing (CA BRN)
  • Valid Public Health Nurse (PHN) certificate issued by the CA BRN
  • Valid Healthcare Provider Basic Life Support (BLS) certificate
  • Three (3) months of full-time equivalent experience as a licensed Public Health Nurse or three (3) months of full-time equivalent experience in home health or home visiting as a licensed Registered Nurse or licensed Public Health Nurse

Preferred Qualifications:

  • Experience working with pediatric populations
  • Enhanced care management experience
  • Certificate or training in social determinants of health
