Job Summary

We are seeking a skilled Nurse Case Manager to join our team at FlexStaff Careers. As a key member of our healthcare team, you will be responsible for coordinating the delivery of routine and specialty health care services to individuals with developmental disabilities.

Key Responsibilities
  • Assess and establish health care outcomes for individuals receiving services, updating assessments as needed.
  • Coordinate with Medical Case Manager Counselors to ensure medical, dental, and specialty appointments are tracked and scheduled according to requirements.
  • Act as a liaison to communicate health information between team members, families, and other service providers.
  • Develop and update plans of nursing service (PONS) for each individual based on their health care needs.
  • Maintain up-to-date health care progress documentation and electronic health records.
  • Evaluate the effectiveness of preventive/ongoing health care and make recommendations for alternative care as needed.
  • Conduct health-related training for individuals receiving services, direct care staff, and other team members.
  • Assign direct care staff to perform health care treatments/procedures after considering their skill/experience and the individual's health condition.
  • Supervise AMAP staff and partner with the Education Department liaison to ensure all AMAP staff are certified and tracked for recertification.
  • Provide nursing coverage on-site, electronically, or via cell phone between 7:00 am - 7:00 pm, Monday through Friday.
  • Participate in meetings and committees as assigned, including IDT, Day Program, Human Rights, and Departmental meetings.
Requirements
  • Maintain current NYS Registered Nurse license.
  • Graduation from an accredited nursing program.
  • Bachelor's Degree preferred.
  • Prior nursing experience and/or experience working with individuals with developmental disabilities preferred.
  • Excellent oral and written communication skills.
  • Competence in computer skills, including Microsoft Office and electronic medical records.
  • Ability to travel to and access multiple locations throughout agency locations.
  • Flexible work schedule with hours and/or days as approved by the Director of Nursing.
Additional Information
