Healthcare Recovery Coordinator

We are seeking a skilled Registered Nurse to join our team as a Nurse Transition Expert. In this role, you will provide specialized care for patients transitioning from critical care units.
About the Position
As a Nurse Transition Expert, your primary responsibility will be to assess, monitor, and treat patients in the step-down unit. You will work closely with an interdisciplinary team to ensure optimal patient outcomes.
Key Responsibilities:
- Assess and evaluate patients' needs and develop individualized care plans.
- Provide education and support to patients and their families throughout the transition process.
- Collaborate with interdisciplinary teams to ensure seamless transitions between care settings.
- Communicate effectively with healthcare team members to facilitate patient progress and optimize outcomes.
Requirements:
- CURRENT RN LICENSE REQUIRED
- BLS (OTHER CERTIFICATIONS AS REQUIRED BY FACILITY)
- TWO YEARS OF RECENT STEP DOWN EXPERIENCE
- STRONG COMMUNICATION AND ADAPTABILITY SKILLS
