We are seeking a skilled Registered Nurse (RN) to join our Emergency Department team at Adventist Health. As a Nurse Care Manager, you will play a critical role in ensuring the efficient and effective care of our patients.
Key Responsibilities- Assess patients and identify options for alternative levels of care, including acute rehabilitation, long-term acute care hospitals, and nursing homes.
- Collaborate with multidisciplinary teams to develop and implement patient care plans.
- Communicate with patients, families, and healthcare teams to ensure seamless transitions of care.
- Utilize InterQual criteria to determine patient status and make informed decisions about care.
- Participate in quality improvement initiatives to enhance patient outcomes and reduce costs.
- Bachelor's Degree in Nursing (BSN) or Master's Degree in nursing or healthcare-related field.
- Registered Nurse (RN) licensure in the state of practice.
- Accredited Case Manager, Certified Case Manager, or Board Certification in Case Management preferred.
