About the Role

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our Home Health team at Beebe Healthcare. As a Home Health LPN, you will play a vital role in providing high-quality patient care and support in the comfort of their own homes.

Responsibilities
  • Administer personal care and routine skilled nursing care to patients in their homes, under the direction of a Registered Nurse.
  • Collaborate with physicians, patients, and their families, nurses, therapists, social workers, and other members of the patient care team to implement and follow a patient-specific care and treatment plan.
  • Assess and provide patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
  • Provide accurate and timely documentation consistent with the plan of care.
  • Participate in Case Conferences and provide direct patient care as defined in the State Nurse Practice Act.
Requirements
  • LPN Program Graduate
  • LPN - Licensed Practical Nurse
  • BLS - Basic Life Saving Certification
  • Valid DL with no more than 3 moving violations
  • Knowledge and experience with Electronic Health Records
