Job Summary:
We are seeking a skilled and compassionate Home Health Nurse to join our team. As a Home Health Nurse, you will have the opportunity to provide personalized care to patients in the comfort of their own homes. Your expertise and dedication will play a critical role in helping us achieve our mission of delivering high-quality, patient-centered care.
Key Responsibilities:
- Patient Care and Service: Provide comprehensive assessments, skilled nursing care, and patient education in accordance with care plans to ensure optimal patient outcomes.
- Planning and Development: Develop and document individualized care plans customized for each patient's unique needs, taking into account their medical history, lifestyle, and personal preferences.
- Communication and Collaboration: Maintain effective communication with office and supervisory staff, as well as coordinate client services and referrals to ensure seamless care delivery.
- Documentation and Compliance: Accurately document client care and implement physician orders, adhering to department and system protocols, regulations, and education requirements.
- Professional Development: Stay up-to-date with the latest clinical practices and guidelines, participating in ongoing education and training to enhance your skills and knowledge.
Requirements:
- Education: Associate's degree in Nursing (Required)
- Licenses/Certifications: Basic Life Support (BLS) certification and Registered Nurse (RN) licensure
- Physical Demands: Medium Work - Exerting 20 to 50 pounds of force occasionally, requiring frequent walking, standing, or squatting
Compensation and Benefits:
We offer a competitive salary range of $70,000 to $94,300.00, as well as a comprehensive benefits package, including health and wellness benefits, retirement savings programs, and more.
