Key Responsibilities
  • Family Progress - Monitoring and Coaching (50%)
    • Collaborate with families to develop and implement personalized plans to achieve their goals
    • Provide regular home visits and coaching to support family progress
    • Assist families in accessing community resources and services
    • Attend court hearings and provide recommendations to support family stability
    • Communicate effectively with schools, social services, and other stakeholders to ensure comprehensive support
  • Service Plans / Goal Development (20%)
    • Develop and update service plans to meet the unique needs of each family
    • Build relationships with community agencies and organizations to ensure access to resources
    • Work collaboratively with other social welfare agencies to provide comprehensive support
  • Intake and Needs Assessment (15%)
    • Conduct thorough intake assessments to identify family needs and develop personalized plans
    • Visit families in their homes to gather information and assess their needs
    • Ensure that all assessments and plans are completed in accordance with Firefly Children and Family Alliance standards
  • Reporting and Documentation (15%)
    • Maintain accurate and timely records of family progress and service plans
    • Prepare written reports for the Department of Child Services and other stakeholders
Requirements
  • Bachelor's degree in social work or a related field
  • At least 21 years of age
  • Valid driver's license and insurance with no more than 6 points on driving record
  • Ability to travel between appointments to serve clients in their community
  • Knowledge of social service systems within the state
  • Understanding of confidentiality practices and cultural sensitivity
