Key Responsibilities- Family Progress - Monitoring and Coaching (50%)
- Collaborate with families to develop and implement personalized plans to achieve their goals
- Provide regular home visits and coaching to support family progress
- Assist families in accessing community resources and services
- Attend court hearings and provide recommendations to support family stability
- Communicate effectively with schools, social services, and other stakeholders to ensure comprehensive support
- Service Plans / Goal Development (20%)
- Develop and update service plans to meet the unique needs of each family
- Build relationships with community agencies and organizations to ensure access to resources
- Work collaboratively with other social welfare agencies to provide comprehensive support
- Intake and Needs Assessment (15%)
- Conduct thorough intake assessments to identify family needs and develop personalized plans
- Visit families in their homes to gather information and assess their needs
- Ensure that all assessments and plans are completed in accordance with Firefly Children and Family Alliance standards
- Reporting and Documentation (15%)
- Maintain accurate and timely records of family progress and service plans
- Prepare written reports for the Department of Child Services and other stakeholders
- Bachelor's degree in social work or a related field
- At least 21 years of age
- Valid driver's license and insurance with no more than 6 points on driving record
- Ability to travel between appointments to serve clients in their community
- Knowledge of social service systems within the state
- Understanding of confidentiality practices and cultural sensitivity
