Job Summary

We are seeking a highly skilled and compassionate Home Health Registered Nurse Case Manager to join our team at Signature Healthcare at Home. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and managing patient care services in a home setting.

Key Responsibilities
  • Provide skilled nursing services to patients in their homes, including wound care, medication management, and education on disease management.
  • Develop and implement individualized care plans that meet the unique needs of each patient.
  • Collaborate with healthcare providers, family members, and caregivers to ensure seamless care coordination.
  • Conduct comprehensive assessments of patients and their living situations to identify care needs and develop strategies for improvement.
  • Document patient progress and outcomes in a timely and accurate manner.
  • Participate in interdisciplinary team meetings to discuss patient care and share best practices.
  • Stay up-to-date with industry developments and best practices through ongoing education and training.
Requirements
  • Graduate of an accredited school of nursing with a current RN license in the state of practice.
  • At least one year of experience in home health or acute care nursing, with a strong background in case management and patient education.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Strong problem-solving and critical thinking skills, with the ability to analyze complex patient situations and develop effective solutions.
  • Ability to work independently and as part of a team, with a strong commitment to delivering high-quality patient care.
