Position Overview:
As a vital member of our healthcare team, you will provide comprehensive client services to local Skilled Nursing and Rehabilitation facilities. Your role will ensure the delivery of high-quality nursing care by directing and coordinating patient care activities.

Key Responsibilities:
  • Conduct physical examinations and annual assessments for assigned patients.
  • Perform acute visits to evaluate and diagnose patient conditions.
  • Emphasize early detection of condition changes and effective management of chronic illnesses.
  • Complete history and physical evaluations for new admissions within the required timeframe.
  • Prepare discharge orders, instructions, and prescriptions for patients transitioning out of care.
  • Facilitate advanced care planning and end-of-life discussions for patients in need.
  • Provide necessary narcotic prescription refills as dictated by patient requirements.
  • Document patient assessments and care plans in the electronic medical record system.
  • Review laboratory results and diagnostic tests to determine further interventions.
  • Collaborate with physicians and healthcare staff to ensure optimal patient outcomes.
  • Communicate significant changes in patient conditions to family members.
  • Engage daily with nursing staff, assigning tasks, supervising, and evaluating performance.
  • Review nursing policies and procedures for compliance and recommend necessary updates.
  • Conduct or recommend specific areas for in-service training or continuing education.
  • Assess patients for palliative care needs and manage the palliative care plan in facilities and home settings.

Qualifications:

Current, unrestricted Nurse Practitioner license from the Board of Nursing or Physician Assistant license from the Medical Board in the state of assignment, or the ability to obtain such licensure.
DEA certificate current or the ability to obtain.
Preferred clinical background in adult, family, or geriatric specialties.
Experience in long-term care, nursing homes, or with senior populations is preferred.
Strong communication and relationship-building skills are essential.
Intermediate proficiency in PC-based word processing, including Microsoft Word and Outlook.
Willingness to be 'on call' for facilities as needed.
Dependable, flexible, and able to collaborate effectively with nursing personnel and other departments.
Ability to make informed decisions regarding nursing issues, perform well under pressure, and solve problems efficiently.
Background checks/drug-free workplace.
